Whether you are ordering Sight for Student gift certificates for the first time, or you are
reordering gift certificates, you must fill out VSP's "Easy as 1,2,3" form.
To order the form please submit an e-mail request to sfsinfo@vsp.com.
Be sure to include your name, address, the name of your qualifying organization and a
fax number. After we receive your request, we will fax you a copy of the 1,2,3 form.
Simply fill it out and fax back to us at the number listed on the form. It takes us about
three weeks to process your order, verify your information, print the certificates and mail
them to you.
